If you are a County Probation Officer and wish to attend a class, please read the County Probation Officer Letter which outlines the requirements for your attendance. Please have all documentation to MTU 9 by Deadline on Flyer -- ORIGINALS ONLY -- NO COPIES (Copy of Form E is an exception)

Blank course sign-in form for use by MTU regional departments who wish to report any in-service training to our office for certificate and credit.  Everyone in attendance must sign in on this form.

Course Evaluation Form

Blank evaluation form for use by MTU regional departments who wish to report any in-service training to our office for certificate and credit. We should have an evaluation form for each person listed on the sign-in sheet.
Course Release Agreement

- Blank release agreement form for use by MTU regional departments who wish to report any in-service training to our office for certificate and credit. This form must be filled out in its entirety and signed by each officer in attendance of any course where physical injury is a possibility.

This form must be completed by every instructor who teaches through MTU 9.  Please email completed form to director@mtu9.org along with course description, course outline, specific mandates being taught within course and the time allotted, and include any Instructor course certificates or certification that you have aquired.
